A Clinical Analysis of Operation for 15 Cases of Acute Pancreatitis



Abstract
The clinical study was made on 15 cases of acute pancreatitis who were operated at department of surgery, Chung Goo Sung Sim Hospital from August 1988 to December 1991 and the following results were obtained.
1) The male to female ratio was 6.5:1. The peak incidence was in the age group of 3-~40 years.
2) The causative factors were excessive alcohol intake in 9 cases(60.0%) biliary tract disease in 5 cases(33.3%), hyperparathyroidism in one cases (6.6%).
3) Serum amylase was the most sensitive laboratory finding showing variable range in 125~680U/L. The peak level of serum amylase was 400~600 U/L in 8 cases953.3%) and overall mean level was 483 U/L.
4) The operative procedures were Gastrostomy with sump drainge in 7 cases946.6%), drainage only in 2 cases(13.3%), cholecystectomy with choledochoduodenostomy in 2 cases(13%), cholecystectomy with T-tube drainage in 2 cases913.3%),
cholecystectomy
with
transduodenal sphincteroplasty in 1 case96.6%0 triple ostomy in 1 cases96.6%).
5) The postoperative complications were pulmonary complication in 7 cases, wound infection in 5 cases, ileus in 4 cases, pancreatic abscess in 2 cases, pancreatic pseudocyst in 1 cases, enterocutaneous fistula in 1 cases.
6) Total parenteral nutrition was performed via subclavian catheterization in all cases. The mean duration of the hyperalimentation was 22.9 days.
